DS-3053 Passport Consent NotarizationA little preparation. A smoother departure.

If a parent or legal guardian cannot attend a child’s passport application appointment, the U.S. Department of State may require a DS-3053 statement of consent. We meet the consenting parent or guardian in Bucks County to notarize their signature on the prepared form.

What to expect

At your appointment

The consenting parent or guardian attends with photo ID and signs before the notary. This visit is for consent notarization, not passport application acceptance.

More about the appointment

The parent or guardian signing the consent appears in person with acceptable photo identification. We administer the required oath or affirmation, witness the signature and complete the notary section. This appointment is for consent notarization; it is not a passport application acceptance appointment.

Signing and witnesses

Sign together with the notary

DS-3053 specifically requires signing when directed by the notary or passport authorizing officer. The signer and notary date the form on the same day. Do not pre-sign this form.

Consent and custody questions

A notary cannot decide who has legal authority to consent or whether a different passport exception applies. Ask the Department of State about application requirements and an attorney about custody questions. The notary cannot be related to the consenting parent or guardian.

Our role

We handle notarial acts and signing logistics. Bring prepared documents and instructions; we cannot draft documents or give legal advice.

What notarization does—and does not—cover

Bring prepared documents and instructions from your document preparer or receiving institution. We perform requested notarial acts and coordinate signing logistics. We do not draft documents, select legal instruments or notarial certificates, interpret legal effects, or provide estate planning or legal advice. Notarization does not guarantee a document’s validity, enforceability, or acceptance.

Before you book

Your questions, answered.

The person signing the DS-3053 must appear before the notary. The child’s separate passport application has its own appearance requirements; follow the Department of State’s instructions for that appointment.
Do not assume a travel letter replaces passport consent. These serve different purposes. Use the form or evidence required by the Department of State for your situation.
No. The Department of State determines whether the application and consent satisfy its requirements. We complete the requested notarial act, not the passport review.
